COVID-19 Self-Report Vaccination Portal

You can now self-report your COVID-19 vaccination status using the below form. Submitting your card helps us track and keep an accurate percentage of vaccinated employees across all Skookum sites. For your benefit, if you happen to lose your vaccination card, Skookum will have an electronic version for you. Your vaccination status remains confidential to our Human Resources department and your Site Manager.

Please be sure to submit a copy or photo of the FRONT and BACK of your vaccination card. Please be sure to update your records by showing proof of vaccination for each dose you receive. If you have only received your first dose (Pfizer or Moderna), you can still submit your card and then resubmit again after receiving your second dose.
